Fatal scaffold fall

A 65 year old man has died following a fall from scaffolding in Monmouth, Wales in late May.

The man David Llewellyn was working on a farm near the town on Wednesday May 25th, when he fell from a scaffold that he was using to work at height.
Rapid response cars, an emergency ambulance and two Air Ambulances were sent to the scene along with the police and fire service, but Llewellyn was declared dead at the scene.

A joint investigation between Gwent Police and the Health and Safety Executive is ongoing.
