Two accidents in W.Australia

A man in his 50’s was electrocuted this morning in Attadale as he trimmed trees from a boom lift. Local reports say that the man cut into a high tension overhead cable with his chainsaw and died instantly.

In another unrelated accident this morning at the Wedgefield industrial precinct in Port Hedland a 25 year old man was working on a silo from a JLG 800AJ articulated boom lift around six metres up, when according to reports the lift collapsed underneath him.
Worksafe the Australian health and safety organisation has sent inspectors to the scenes to investigate

Update:

The superstructure of the AJ800AJ became detached clearly through the failure of slew ring bolts a very unusual cause for failure.
