A man died last Monday - Septermber 14th - in the Ostia suburb of Rome, following a structural failure on a small truck mounted work platform.
Two men had been working at a height of around five or six metres from the platform of an ageing Oil & Steel 3.5 tonne articulated truck mounted lift, when something broke within the sigma type dual riser lift mechanism. The top arm crashed onto the chassis cab, and the men were both thrown out of the platform.
The man who died, Daniele Bellisari, 46, landed on the sidewalk/pavement and died instantly. His colleague somehow managed to grab onto a balcony as he fell and was rescued by neighbours and taken to hospital.
The Carabinieri and SPRESAL, the safety inspectorate, are investigating the incident along with the ASL local health authority. The lift was impounded and taken away for analysis, while investigators stated that harnesses and lanyards had not been used.

Sadly, both men might have lived if they had been wearing a harness with a short lanyard attached. As to the machine, it certainly looks, from the photos and information we have received, like it has seen better days.
