Plank stupidity
The operator of a mast boom working at a supermarket in Strood, south east of London, UK, drove the machine onto an elevated boardwalk that runs alongside the river last week, with the obvious result that it crashed through the floorboards and overturned.
The lift, a Haulotte Star 10 from the HSS Hire fleet, weighs around 2.7 tonnes, and has small solid wheels/tyres which can apply high point loadings. To make it worse the operator drove it counterweight end first onto the timber walkway - when stowed the machine is naturally back heavy.

Thankfully no one was hurt in the incident and a statement from the supermarket - Morrisons - said: "During some routine maintenance work, a cherry picker damaged a bridge. No one was hurt in the incident and the area has been safely secured until a full repair can be completed."

I bet the operator didn't have a card or training. This should not be happening in today's world.

The Morrisons response is somewhat misleading. An idiot operator damaged a light walkway (bridge if you prefer) by driving an AWP onto it, with little if any forethought of either the weight of machine or capacity of the structure.
I'm guessing no harness was involved, and from the angle the machine finished in, the operator probably ended up in the water (one can only hope!)
