Fatal mastclimber incident
Three men died on Monday morning when a mastclimbing work platform collapsed in Raleigh, North Carolina.
The mastclimber had been working on an 11 storey building and was being dismantled at the time. The failure reportedly occurred around a building tie on the fifth floor.

Police said that the deceased were Jose Erasmo Hernandez, 41, Jose Luis Lopez-Ramirez,33 and Anderson Almeida, 33. Another man working on the platform when it collapsed - Elmer Guevara, 53, was seriously injured, is fall was broken when he landed on the plastic roof of a portable toilet.

At least one of the deceased worked for Associated Scaffolding which had supplied the mast-climber to the main contractor on the project Choate Construction, while the others are reported to have been employed by Juba Aluminum Products.
OSHA is investigating and we will update this article when we receive more information
