Crane overturn in Gdansk
A dockside crane overturned in the Polish port of Gdansk, yesterday injuring the crane operator and another man.
The incident occurred at the Wisla shipyard where witnesses say that the crane began to tilt slowly and then went over, and coming to rest on the hull of a ship that is under construction. Firefighters were on site rapidly and managed to evacuate an injured man form the ship, before extracting the crane operator from his cab.

The overturned crane
The man on the ship suffered injuries to his head and hands, while the operator suffered foot injuries. Both were rushed to hospital for treatment. An investigation was launched within a few hours of the incident occurring.

I do not understand this kind of accidents. As murphys law says: When it can go wrong, ... everything goes wrong.
1. check that the automatic safe load indicator and work limiters are correctly set;
2. check traveling working space limiter/anti-collision system;
3. check rail-mounted wheels and axles and bogies are in good condition;
4. check all rail clamps
5. check track is in good condition and clear of obstructions, loose joints, cracks, or gaps between adjacent length of rail;
6. check load
Those procedures should be a standard daily checkup for a compenent crane driver. Incidents like this may not happen when a crane is periodic inspected and maintained properly.
