Outrigger incident in NZ
An All Terrain crane lost stability last month while carrying out a small lift on the Puhoi to Warkworth road bridge project north of Auckland in New Zealand.
The incident, which occurred on September 24th, involved a Liebherr LTM 1090-4.1 owned by New Zealand Crane Hire that was set up on soft ground with relatively small mats/cribbing under the rear outriggers. The crane appears to have its full 50 metre boom extended and at a fairly low angle - it was lifting just 150kg at the time - but it also only had a relatively small counterweight configuration installed.

The operator stayed in the crane until he had an opportunity to get out safely, no one was injured, and the damage does not appear to be significant.
The contractor carried out an investigation and published a safety reminder bulletin which said that its initial investigation determined the crane was set up on a non-approved work platform/base. It has indicated that all lifts where outriggers are used, require a permit which includes a ground stability check in advance of setting up.

I can't find any comments in the investigation about the use of proper outrigger mats...
